STEP 3: During your call with our office, we ask to email or fax a copy of your insurance card to verify your benefits. Fax to 703-941-4361 or email to verifybenefits@octainmd.com

STEP 4: We will confirm your appointment. Please contact us within 24 hours to cancel appointment. A $50 cancellation fee will be assessed if appointment is not cancelled within this time frame.

Please arrive early so that we can complete your initial paperwork. Also, please bring the following:

  • Patient’s insurance card and picture id.
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient’s medical and surgical history
  • Recent test results, x-rays, or relevant record
